Incident summary – Minutes after take-off from Pucallpa, the helicopter appeared to have a problem en route and smoke was coming from the aircraft.  Shortly after it crashed into a wooded area killing all 7 on board – 2 Peruvians and 5 Americans .   It was reported that most worked for Petrominerales – for whom the helicopter was being operated
