12-Mar-13 Bell AH-1 Shfela, Israel (2F)

Accident around 1am local on night training mission.  Both reservists killed, named by IDF as Lt.-Col. (res.) Noam Ron, 49, and Maj. (res.) Erez Flekser, 31. The pilots had radioed the Palmahim air base before the crash, saying they would land there in six minutes. There was no distress call prior to the accident.

the helicopter was taking part in the filming of an episode of “Top Gear-Korea” when it crashed.  Initial reports indicate the crash was caused by a mechanical malfunction at a low altitude.  2 POB were reported safe soon after the accident.  A video was posted on LiveLeak.com from inside the cockpit of the whole sequence

19-Sep-11 Bell AH-1W Camp Pendleton, US-California (2F)

The helicopter went down at approximately 1:00 p.m. Monday in a training area in the southeast corner of Camp Pendleton.  A small brush fire broke out as a result of the crash. Fire crews are on scene working to contain the flames.  The two fatalities were later named as Capt. Jeffrey Bland, 37, and 1st Lt. Thomas Heitmann, 27

30-May-11 Bell AH-1 Isfahan, Iran (2F)

A military helicopter belonging to Iran's Air Force has crashed near the central city of Isfahan, killing the pilot and co-pilot, a senior Iranian official says.  Deputy Governor of Isfahan Mohammad Mehdi Esmaili said that the crash was caused by technical problems in the helicopter.
