We have a recent report in of an emergency landing at 10.28pm of an EMS helicopter near the junction of New River and Cross Mountain roads in New River, Arizona - northern outskirts of Phoenix. Air Methods operating for "Guardian Air"
Three killed in night time accident north of Ventura - pilot Gene Grell, nurse Shelly Lair-Langenbau and paramedic Russell Piehl. Reports state it was the helicopter based at Mercy Medical Center-North Iowa - which our records show to be Bell 407 N445MT (see photos). Post impact fire. Was en route from Mason City to Emmetsburg.
Damaged in emergency landing near Vaishno Devi shrine - remaining mostly undamaged and relatively upright into the trees/shrubs into which it fell. Minor injuries to 7 POB, named as pilot A S Parmar and passengers Anita Puri, Meenakshi, Arun Kumar, Arti Devi, Raj Rani and Chetan Kumar. Sources vary, but most report a technical issue shortly after take-off and the pilot electing to put the helicopter down asap.
Force landing after striking power lines. Wire strike system successfully cut cables and helicopter landed safely. This report suggests that the helicopter was flying at 80-100 ft altitude, and thus the crew "were acting outside the scope of their employment and in violation of Federal Aviation Regulations, company operational specifications and policies."
Chin window damaged by patient trolley and helicopter taken out of service until new parts fitted. Life Link III operate five Bell 407s - N404LL through N408LL so presumably one of these was the subject aircraft
Crashed into lake after contacting zip wire which spans the lake. One drowned (named as Daniel Segundo Martinez), four survived. Helicopter was being used to capture video footage of the lake for use on a television program
State Police Trooper/pilot Trooper Thomas A. Shultz, 41, was injured in a very heavy landing after "he experienced an in-flight mechanical emergency." Photo shows skids splayed out after landing in a wooded area
Crashed into lake shortly after take-off around 10.20pm - after dropping off six passengers at helicopter owner's home. Sole occupant pilot was named in local press as Bill R. Starnes, 64 (see obituary)
Helicopter went AOG for unknown reason and was roaded out from field landing. We are 99% sure this was N911KX, although the press photo shows the registration etc all covered up. Compare to this photo of N911KX which is known to be based in Iowa.
Helicopter was fire-bombed while parked on the helipad outside Verde Valley Medical Center. Alleged that a beer bottle filled with fuel was thrown into cockpit - photos show extensive fire damage in that area
While en route to a medical emergency at Steele, LifeSaver 1 from Birmingham was struck by a goose and landed in the parking lot at Margaret Elementary School.

On February 21, 2012, about 1400 central standard time, a Bell 407 helicopter, XA-JMB, had a loss of engine power and impacted terrain near Loma Bonita, Veracruz, Mexico. The commercial pilot, who was the sole occupant, was seriously injured. The helicopter was substantially damaged and was operated by Heliservicio Campeche S.A. de C.V. Visual meteorological conditions prevailed. The local flight and had departed from Veracruz, Mexico.
Damaged in hangar prior to being put into service. Fixed wing aircraft manoevred and struck helicopter twice, causing two long scratches, which are reported as "relatively minor" were then covered up using a marker pen. All activity caught on hangar video surveillance. Gadsden Police on the case
Fatal crash while on geological mission on Nelson Glacier. Aircraft remains found at 5,100ft altitude. Victims later named by RCMP as 35 year old pilot Randy Lambert from Abbotsford, 40 year old Colin Dionne from Powell River and 32 year old Mathieu Lefebvre-Masse from Lund BC
Helicopter was on electricity line survey when it went down at the crest of the mountain of Puerto Angulo, in a rocky and inaccessible by land vehicle. Locals reported a fire and the authorities later confirmed two fatalities. Foggy weather conditions may have contributed
Precautionary landing in field. FAA preliminary report quotes registration as "Wings 1" which would be the callsign. Wings Air Rescue operates four Bell 407s (N407WA, N911WA, N511MT and N513MT) and we expect the subject one here to be N407WA but we have not got 100% confirmation on that yet
The Interior Ministry says six firefighters have died when their helicopter crashed in a remote region in the northeastern province of Aragon as they flew to try and put out a wildfire. AirMed was responding to a rollover on I-84 when the rotor struck a metal pole on the side of the road. The craft raised 10 feet in the air and began tipping on its side before the pilot brought it down hard on both skids, which splayout out.
