Post maintenance test flight.  Cameron County Sheriff identified the deceased as pilot, William R. Croucher, 69, and two mechanics: Michael Lee Tyree, 55, and Timothy Lloyd Goehring, 41.  Operator Era Helicopters made a brief statement confirming the accident

12 December: Bond Offshore Helicopters can confirm that one of its aircraft developed a technical problem whilst on an offshore platform. Engineers were dispatched and examined the aircraft. Once the relevant part has been delivered and fitted, the aircraft will be returned to normal service.  update 14th December - G-PERA returned to Norwich Airport yesterday afternoon after the required part was fitted. Bond then carried out five additional flights yesterday, returning 75 passengers to Norwich Airport

At 50 feet, during the demonstration of Class B landing by the training captain, the helicopter experienced a sudden drop in height, hitting the ground nine metres short of the runway. There was no fatality, but damage to the nose wheels, the left main landing gear forward structure and the tail guard.
