While conducting aerial agricultural operations, the helicopter struck a wire and subsequently collided with terrain. The helicopter was substantially damaged and the pilot received minor injuries.

Crashed on powerline patrol, killing both on board – 51-year-old pilot Matthew Wallace of Hiram, Georgia and 63-year-old David Carson of Tuscaloosa, Alabama who was employed by Alabama Power.

On a local flight 32 NM northwest of Matagami, QC, when the engine (Honeywell LTS-101-700-D2) failed on final for the helipad. The aircraft was not damaged and the pilot, alone on board, was not injured. The TSB will implement a plan to examine some of the components CADORS 2014Q1697

Mil Mi-8MT helicopter of 16 Army Aviation Brigade was shot down by Donetsh People Republic fighters. Helicopter crashed but crew managed to leave the downed machine and return to air base. The next day the same crew flying on another helicopter would be shot down again. (see HeliHub.com page for that accident).

Heliski helicopter crashed on mountain.  Boom detatched from cabin, which rolled further down the slope.  1 fatal (named as Jerome Box) of 7 on board – five skiers, their guide and the pilot. The other six all to hospital, none with life-threatening injuries

Helicopter came down with significant vertical force, causing the skids to splay out, the cabin to touch the ground and the boom to detach at the root.  That said, the main and tail rotor blades were not damaged.  4 of 6 POB injured

Dale Elio, 54, was crop-spraying herbicide on a bean field when main blades contacted power lines.  Helicopter hit ground nose-first, then fell on its starboard side.  Minor injuries.  Brief power outage from broken power lines.

Crashed into a lake/swamp with 4 POB, all out OK. Helicopter on its starboard side in water less than 2m deep with port skid above the water level. Pipeline survey flight, low level.  One report suggests all occupants had recently been drinking alcohol

Crashed west of Whitecourt, AB after experiencing tail rotor problems. Four occupants suffered minor injuries, two were taken to hospital. The aircraft had significant damage.  Helicopter was contracted to Alberta Environment and Sustainable Resource Development and carrying out a pine beetle survey CADORS 2014C3111
