09-Feb-19 ET2015 Mil Mi-17V-5 Abyei, South Sudan (3F)

Ethiopian Air Force Mi-17V-5 being operated for the United Nations Interim Security Force for Abyei (UNISFA) crashed inside the UNISFA compound.  The aircraft was on routine operation carrying Ethiopian troops on rotation from Kadugli to Abyei, and it could be assumed it was intending to land in that compound when it experienced some sort of difficulty and came down in the wrong place.  The helicopter hit the ground very heavily and came to rest upright, breaking the tail boom in two and all main rotor blades losing most of their length.  An alarming video shows rescuers going in and pulling at least one person out while the main rotor head continues to turn under power.

Of the total 23 on board, three died in the accident and ten passengers were injured with three in critical condition.

NOTE – Registration being wrongly quoted (including by the UN here) as UNO370P, which is the UN operational code and not the registration.  The military registration/serial carried on this helicopter was ET2015, per this image.

While involved in flood relief support, helicopter landed in a residential garden, with the boom breaking off against the roof of the house. Registration validated by photo on this page

None of the 11 POB injured.  Helicopter substantially damaged

37-year- old SLAF airman, YMS Yaparathna, fall off a helicopter while conducting a rescue operation, along with the two individuals he was rescuing.  All three were injured when they fell, and were admitted to the Karapitiya Teaching Hospital where the airman later died of his injuries.  He was posthumously promoted from Flight Sergeant to the rank of Warrant Officer

Crashed in unknown circumstances.  Helicopter was flying between Zabul’s capital of Qalat and the town of Shinkay, 20 kilometers (12 miles) away. Accident killed all on board –  12 soldiers and five crew members. The Taliban claimed they had shot down the aircraft

06-Aug-15 Mil Mi-17 Lassan Nawab, Pakistan (12F)

The helicopter, involved in medical evacuation work after recent floods, was en route from Rawalpindi to Gilgit when it crashed near Lassan Nawab. Local media reported that weather was a contributory factor, and noted that there was a post-crash fire. The deceased were named as Major Humayun (Pilot), Major Muzammil (Pilot), Major Shahzad (Doctor), Major Atif (Doctor), Major Usman (Doctor), Havaldar Munir Abbasi, Nursing Assistant Amanullah, Naik Amir Saeed, Naik OT Maqbool, Sepoy Rehmat-ullah and Sepoy Waqar.

06-Aug-15 EP-662 Mil Mi-17 Piura, Peru (1F)

Student Dioselinda Zapata Feijó was walking near the helicopter shortly after it landed was struck fatally by the tail rotor, after reportedly ignoring the security cordon. Municipal official Kathia Izquierdo said the woman’s brother reported she was trying to deliver a letter to the Cabinet chief asking that a relative be freed from prison.

06-Aug-15 Mil Mi-17 Khot, Pakistan

Pakistan Air Force helicopter, engaged in relief and rescue efforts, crashed in the Khot area of upper Chitral, injuring several people. Media reports say the blades hit a tree, un-stablilising the helicopter on take=off, and it subsequently hit a house and crashed.

During a display of the “Golden Eagles” display team, one of the helicopters started to spin on its axis and gradually descend until it hit the ground hard, killing the pilot and seriously injuring the co-pilot.  Post crash fire

EMS flight in poor weather conditions, crashed killing two medical staff, four crew and the baby patient, only a few days old.  A later report indicated “The post mortem showed there were 0.68 milligrams per milliliter of alcohol in Mehic’s blood, and 1.30 mg/ml in his urine.”

04-Sep-14 RA-24255 Mil Mi-8 Gelendzhik, Russia (2F)

Crashed near Gelendzhik airport and caught fire. Full video of accident here.  From a steady hover the helicopter lost height and hit the ground heavily and bounced.  The impact caused the tail boom to shear off and thus the helicopter became dynamically unstable, it immediately crashed onto its belly and caught fire.  It burned out completely.  Both occupants died – noting that the video does show one person being taken by stretcher to an ambulance.

19-Aug-14 Mil Mi-8 Donetsk, Ukraine

Mil Mi-8 helicopter was shot down by Donetsh People Republic fighters. Helicopter crashed and crew member were severely injured. Later they were evacuated to hospital. The very same pilots flew on Mil Mi-8MT, 62 YELLOW which was shot down the day before (see HeliHub.com page for that accident). That time they were into luck to escape uninjured.

18-Aug-14 62 Yellow Mil Mi-8MT Donetsk, Ukraine

Mil Mi-8MT helicopter of 16 Army Aviation Brigade was shot down by Donetsh People Republic fighters. Helicopter crashed but crew managed to leave the downed machine and return to air base. The next day the same crew flying on another helicopter would be shot down again. (see HeliHub.com page for that accident).

Helicopter was involved in evacuating people in a humanitarian emergency situation in northern Iraq.  Crashed – killing the pilot (reported as “Majed”) – possibly due to overloading. Those on board included

  • Yazidi member of parliament, Vian Dakheel
  • New York Times reporter Alissa Rubin – see story
  • Australian photographer Adam Ferguson, freelancing for the New York Times
  • Photographer Moises Saman, on assignment for TIMEsee story

One of the (apparently two) Mi-17s involved in this work was YI-403 (see video here).  The ITV reporter quotes one flight of the Mi-17 taking as many as 50 (yes, fifty) refugees on board, desperate to escape their dire situation (see TIME video for typical flight)

Fuad Hussein, the chief of staff for the president of the Kurdistan region, attributed the crash to an accidental loss of control by the pilot when the aircraft hit a boulder as it was lifting off.

Initial reports stated one dead, one survived, one missing presumed dead – but the latter trekked through the bush and back to base

Soldier abseiling down rope died after falling to the ground.  Reports in subsequent days suggest the rope was intentionally cut after the helicopter suffered a major malfunction and had to make an emergency landing to save the 13 on board.
