Crashed on landing – the 68-year old owner of the helicopter Trygve Nils Svard was landing at his son’s house. Helicopter started to spin and it was put down hurriedly, remaining upright but catching fire. Photos show a trailer well within the rotor disc area at its final resting spot. Pilot suffered minor burns only.
Chin window damaged by patient trolley and helicopter taken out of service until new parts fitted. Life Link III operate five Bell 407s – N404LL through N408LL so presumably one of these was the subject aircraft

went down into Ten Mile Lake, near Hackensack, MN killing pilot John Zacher, whose body was found the next day. His wife Vicki Zacher was the only passenger and survived.
Force trim release button would neither release nor respond to input on final approach to scene request. Aircraft declared u/s and patient transferred by land ambulance. Button was found to be   faulty and was replaced

Pilot Kevin Rossan, 35, works for Scott’s Helicopter Services of LeSeuer, Minn., and was contracted by the Metropolitan Mosquito Control District to spray for mosquitoes. Pilot reported engine failure for sudden descent which ended 30-40 feet from the shore of a lake in 12-15 feet of water.
