The pilot was attempting to land on a marked heliski landing site on a ski run called “Nicole”. This was the second landing of the day at this location; the helicopter was carrying 10 guests, 2 guides and the pilot. The first approach was abandoned due to skiers from a previous group waiting on the landing site. On the second approach, visibility was reduced in fog and blown-up snow. The pilot still had the site markers and skiers in sight when it became apparent that the helicopter could not come to a complete hover and touch down would result in a run-on landing. The helicopter touched down on the pilot’s right-hand side due to skiers near the left side of the site. With some forward momentum and sloping ground to the right, the helicopter’s right-side skid dug into the soft snow, resulting in the helicopter rolling over to the right side. The pilot pulled both fire T-handles and exited the aircraft while the guides were assisting guests out from the back seats. There were no injuries or fire.
The Custom Helicopters Bell 206L-1, C-GOFH, was hovering prior to touchdown at a remote camp in north eastern Manitoba when an unsecured canvas portion of a Quonset hut was blown into the rotor disc. The aircraft landed without further incident. The helicopter sustained damage to one rotor blade and will remain on location pending inspection and repair.
Helicopter was conducting fire control work with one crewperson on board. The fire control work included use of a Bambi Bucket to drop water on the fires, slung under the helicopter by a 100 ft long-line. The pilot elected to land the helicopter at Glenbrook helipad to refuel. The helicopter landed with the bucket and line in front of the helicopter, and the fuel drum to the right of the helicopter. The pilot realised that the helicopter’s fuel cap was on the left side and therefore needed to turn the helicopter around to access the fuel drum.
The crewperson exited, stood in front of the helicopter, and took hold of the long-line to ensure it remained clear during the turn. The pilot then lifted the helicopter to about 2 ft above ground level. The crewperson used hand signals to direct the pilot to conduct a right turn, walking to stay in front of the helicopter and remain in the pilot’s sight. After the helicopter had turned around, the crewperson gave the signal to lower the helicopter, which the pilot followed. As the helicopter lowered down, the tail rotor struck the bucket, which was on the ground behind the helicopter. The pilot detected the strike as a vibration through the pedals, and immediately moved the helicopter forward slightly, lowered the collective, and landed.
The tail rotor was damaged; the pilot and crewperson were uninjured.

CHC EC225 made emergency landing following warning light in cockpit. Helicopter was reported as 60 miles out east from Aberdeen to a rig when it turned round and returned to Aberdeen
During the approach, the helicopter pitched forward, and the pilot moved the cyclic aft to compensate for the forward pitch. Shortly thereafter, the helicopter rolled to the right and entered a spin to the right. The pilot then attempted to land the helicopter, but it impacted terrain and rolled onto its left side. Pilot Ed Faris was transported to a hospital for treatment of injuries not believed to be life-threatening. The passenger, 34-year-old Terry Claessen, was treated at the scene.
R22 went down on a training flight from Venice Regional Airport. Female student named here as Agnus Jolumo was airlifted to Sarasota Memorial Hospital with head and facial injuries, while the male instructor Ronald Vasconcelos went by land ambulance with minor injuries to Venice Bayfront Health
FAA quote this as N8560M but that R22 was cancelled from the N register a year prior to this accident and thus we are not quoting a registration or MSN at this time

Aircraft was taken off from Dimapur with 4 passengers onboard and just after 35 minutes of flight the helicopter had made a crash landing. Helicopter rear portion caught fire after landing and destroyed.
A bird is reported to have hit the tail rotor shortly after take-off. While the pilot was trying to make an emergency landing, the helicopter hit power lines, crashed and caught fire. The helicopter was ferrying pilgrims to the Vaishno Devi temple. The seven fatalities were named as pilot Sumita Vijayn from Kerala, Arjun Singh, Mahesh and Vandana from Jammu, Sachin, Akshita (5) and Aaryanjeet from Delhi.
Maj. Gen. Jeffrey N. Colt, commanding general, First Army Division West, released the following statement on Tuesday 24th: “It is with a heavy heart that I announce the death of four First Army Division West Soldiers as a result of a UH-60L helicopter crash that happened Monday evening sometime after 5:49 p.m. The accident is under investigation and the names of the deceased will be released after the families have been notified. I want to extend my deepest sympathies and prayers to the Families and friends of the Soldiers involved in yesterday’s crash.”
US Army Apache crashed after hitting wires on a routine training mission, killing both on board. Post crash fire. Debris scattered across a road 500m from nearest houses. The authorities reported wire among the wreckage and the upper part of a nearby high-voltage tower was damaged.
Crashed on a glacier and fell down a crevasse, killing pilot Mitch Gameren, two Australian tourists Sovannmony Leang and Josephine Gibson and four British tourists Nigel Charlton and his wife Cynthia, Andrew Virco and his partner Katharine Walker, head of radiotherapy at a major trauma centre.
It is of particular note that the two visible main blades in most photos (example) are near intact.
Landing on trolley pad went wrong – aircraft tipped back and hit tail on ground twice, then lurched forward and crashed, breaking off the main blades and tail. Lack of tail rotor then started the fuselage spinning on the ground, which it did dozens of times (see track in this video) before gradually coming to a halt and finally catching fire
Both occupants suffered fatal injuries – named as 65-year-old pilot Bruce Allen Erickson (chairman of American Bank – Montana) and 60- year-old passenger Wayne Frank Lewis, his real estate agent.
Just moments before spinning out of control, the pilot is reported as saying to ATC that the ground handler(s) had not chocked the wheeled landing trolley, and thus the downdraft made it a moving target, and could have been contributory to the accident.
This AS350B3 was registered N711BE. The registration is currently showing in FAA records as reserved by Mr Erickson, but not allocated to a given airframe. This is likely to be an admin “pace” issue. N711BE was previously allocated to Mr Erickson’s Bell 407, which he sold recently
The crew was conducting an approach to a landing zone inside of a public safety training center. During the approach, a towel was sucked up into the Fenestron tail rotor system resulting in substantial damage. The towel came out of an unsecured trailer next to the landing zone.
Loss of T/R drive due to failure of air conditioning drive pulley. Instructor pilot completed successful autorotation. Emergency AD followed for all EC120s fitted with a particular type of air conditioning
The ATSB is investigating a collision with terrain involving a Robinson R22, VH-HWJ, 90km S of McArthur River Mine, Northern Territory, on 12 November 2015.
While conducting aerial mustering operations, the helicopter collided with terrain. The pilot sustained serious injuries and the helicopter was substantially damaged.
Crashed in remote area killing all three on board – owner Richard Green, his wife Carolyn and their travelling companion, John Davis, a renowned documentary film-maker.
Mr Green used his helicopter as a “flying camper van” and frequently went to obscure locations – and this meant that a search party was not launched for over 36 hours after the accident occurred.
On return from a local flight, pilot stabilizes the hovering helicopter, turns round before starting a fast forward about 5 m height. After about 300m the pilot the helicopter pitched up to stop hovering. He then receives a uncontrolled increase in engine speed and fails to reduce power. He loses control in rotation, the right skid struck the ground and the helicopter goes down on the left side. Substantially damaged

We are seeing many reports in the Italian media of an AW609 accident with two fatalities – named here as Pietro Venanzi and Herbert Moran. “on fire in flight between Santhià and Tronzano, the province of Vercelli” seems to be the common link description at this time.
A helicopter carrying two senior officers allied to Libya’s Islamist-backed Tripoli government was shot down in the Al-Maya coastal region west of Tripoli, killing at least 14 of its 23 passengers, a Tripoli military official said. A later report then raised the figure to 18
The pilot reported he was conducting an approach in flat light conditions, visibility was decreasing, and during a turn, the right skid impacted terrain. The pilot reported the helicopter then came to rest on its side and he egressed from the cockpit. Substantial damage sustained to the helicopter. The purpose of the flight was to extract a work crew.
On second attempt to take off, helicopter gained height very slowly and caught some wires, causing it to crash. Three on board injured. Later report gave those on board as Victor Sosa (pilot), Ariel Murillo (copilot), Ariel Soriano (commander), and paratroopers; Oscar Matamoros (captain), Deivy Ayestas, Alfonso Mejia, Daniel Korea and Fabricio Flores.

A 55-year-old male pilot from Campbell River, B.C., and a 35-year-old male passenger from Prince Albert, Sask., were the only 2 POB when it crashed. They were pronounced dead at the scene. Helicopter was contracted by electric utility SaskPower to access and repair power poles and struck a powerline causing the aircraft to crash. Post crash fire completely consumed the aircraft.
Helicopter ditched and sank. All three on board were originally reported as rescued by Moroccan military but that later proved incorrect and there were then fears they had been kidnapped after the helicopter was found with an empty liferaft beside it (report). Helicopter was en route from a military exercise in Senegal, bound for the Canary Islands
Pilot had completed a 360° turn to the right and was commencing with the same exercise to the left, approximately 90° through the turn the skid gear impacted with the ground, the helicopter continue to rotate to the left and tail rotor stinger and lower fenestron fin assembly impacted with the ground, which had caused the entire fenestron assembly to break off and helicopter impacted with the ground after several rotations in the air, 53 m further on. The wind at the time was from the southeast at between 10 to 15 knots, with clear sky conditions prevailing.

The two British fatalitites were named as Flight Lieutenant Geraint Roberts and Flight Lieutenant Alan Scott, while the American pair were named as Major Phyllis Pelky and Master Sgt. Gregory Kuhse
Helicopter crashed into Lake Toba with 5 POB, and one survivor Fransiskus Subihardayan found afloat in the shallow water on the side of the lake two days later. Others on board, missing presumed deceased, have been named as pilot Capt Teguh Mulyanto, crew Hari Poerwantono, and passengers Sugianto (who had chartered the aircraft) and Nurhayanto
Took off from a temporary helipad in Takahama Cho, Fukui Prefecture, Japan at 10:12 Japan Standard Time. During carrying loads externally, wooden frame, about 1.3 m by 0.5 m, was dropped unintentionally at an altitude about 170 m. The frame was found in a car park. No one was injured. The helicopter sustained no damage.
Cougar Flight 232 was shut down upon arrival at the Hibernia Gravity Based Structure (GBS) after experiencing engine fuel bypass indications on both engines. The aircrew awaited the arrival of maintenance crew. Overnight the Aircraft Maintenance Engineers, in consultation with the engine manufacturer, conducted all required maintenance procedures. The helicopter was subsequently flight tested and certified serviceable on the morning of October 7. Cougar aircrew then returned the aircraft to St. John’s without further incident.

Helicopter was involved in installing an antenna, when it crashed and completely burnt out. Most reports say it hit a pole, but not clear if that was before or after it “experienced difficulties”.

R44 with four POB crashed at the AgLand Cooperative fertilizer plant. Images show (1) tail boom sliced by main rotor blades and (2) heavy damage to the front of the cabin and all occupants injured, with two treated locally at Avera St. Benedict Hospital and two with more serious injuries transferred to a more equipped medical center in Sioux Falls. The three pasengers were all AgLand employees. The location is very close to Parkston Municpal Airport
Crashed in a nose-down attitude during firefighting ops. 2 deceased – reported as a 47 year old female pilot with 5000 hrs experience and 31 year old engineer
Took off from a temporary helipad about 11:00 Japan Standard Time to carry liquid concrete in the externally slung bucket with a cargo hook. After unloaded the liquid concrete, the pilot mistakenly operated the cargo hook release switch and the empty bucket, the weight of which was about 210 kg, was dropped unintentionally at an altitude about 240 m AGL. No one was injured. The aircraft sustained no damage.
Encountered problem (variously reported, but likely a tail rotor problem) at low level while on powerline work. Crashed into residential area. Pilot injured, passenger seriously injured. Helicopter was intentionally equipped for powerline work (see this video)
Helicopter was landing at a gravel pit near a hunting camp. While manoeuvring to land to refuel the helicopter, the tail rotor struck a fuel drum. The rotor blade was destroyed, the tail rotor drive train was damaged and one main rotor blade was damaged by debris. The helicopter landed safely and the pilot, the sole occupant, was not injured.
The pilot was engaged in a private flight, and was accompanied by his wife. After activating the air-conditioning unit during their return flight to FAGC smoke started entering the cockpit/cabin area. The pilot, who had identified an open area below entered into auto-rotational flight in order to land as soon as possible. As they neared the ground he note a game fence in front of them. In order to avoid colliding with the fence he pulled back on the cyclic and allowed the tail stinger to impact hard with the ground, which caused the aft tail boom tail to twist to the left (looking from the aft position) and the tail rotor blades to make contact with the ground, with one of the blades separating from the tail rotor hub assembly. It was found that the air-conditioning drive belt had melted and smoke from the belt was sucked into the cockpit/cabin area.
During a transferring flight of “Odebrecht” Co. personnel, from Quillabamba to Cusco, crossing the Andes Mountains, the Helicopter crashed on a slope surrounding the “Veronica” snowy, destroying it completely. The wreckage was found at an altitude of over 4000m
All five on board died, identified here as Miguel Angel Pinto Panta (pilot), Felix Cardenas Gonzales (airline mechanic), Juan Carlos Lezama Mucching (mechanical Los Andes), Luis Garcia Villanueva Hoses and Nolberto Retarnozo (Odebrecht workers).
Operator Heliflight was reported here as saying the aircraft had 1422.5 flying hours
Helicopter was hovering approximately 40 feet AGL to assess the landing area. The helicopter moved laterally to the right in order to increase the visibility of the landing area. During this time, the helicopter rotated to the right and started to spin about the vertical axis. An attempt to control the rotational spin was made, however, there was little to no tail rotor effectiveness. CGNET was put down after the second rotation while striking the tail rotor on the ground and damaging the landing skids. There were no injuries
“Lost power on a training flight” says the news reports. What the photos show is the helicopter ended up in a private garden with the port skid collapsed and the tail boom sliced by the main rotor close to the tail rotor gearbox.
Helicopter took off at 00h15, the last radio communication with ATC was at 00h30, the flight crew reported to the ATC operation normal. At about 02h00, ATC unable to contact with the helicopter, the dispatcher reported that the helicopter was operating normal. At about 04h32, the CAAC regional office received a report by the local people that a helicopter crashed and caught fire
During an aerial transport, the crew wanted to land near the station of the Bannalp cableway to pick up a container deposited there. During the approach, the helicopter’s tail rotor came in contact with a cable of a small transport cableway. The pilot was able to land the helicopter.
Two of Tignes ski patrol were seriously injured in the crash at 8.15am. There were four trackers on board, in addition to the pilot and a mechanic, to be dropped to reactivate the remote avalanche triggering devices after maintenance period occurred this summer.
A total of five helicopters were damaged by rocks while parked due to an uncontrolled blast 100m away from the pad. Two needed roading to Helitrans main base in Værnes including LN-OFC
